For an older person, it is more often about staying safe, comfortable and connected in a home they have lived in for decades.

Turning eighteen and moving to adult social care
The move out of children's services is one of the most stressful points a family goes through. Support that has been in place for years changes, new assessments appear, and the people you have dealt with are replaced.
We support young adults through that transition and afterwards. That usually means keeping routines steady while everything around them changes, and being one consistent thing during a year when very little else is.
If a transition assessment is coming up, talk to us early. It is far easier to build a package alongside the plan than to fix one afterwards.

What age do you support from?
Eighteen. We support adults of any age above that, and we do not provide care for children or under eighteens.
Do you support the move from children's services to adult care?
Yes. Talk to us before the transition assessment if you can, since it is much easier to build a package alongside the plan than to rework one later.
Can you support someone in supported accommodation?
Yes, as well as in a family home or their own flat. We will work alongside whoever else is providing support there.
How do you keep care age appropriate for a younger adult?
By matching support workers on personality and interests, and by planning around what the person wants to achieve rather than a standard visit template.
Do you support people with physical disabilities?
Yes, at any age, including mobility support and transfers where our carers are trained for the equipment involved.
Can care change as needs change?
Yes, in both directions. For older clients packages usually grow. For young adults, a successful package often shrinks over time.
