Care at home, for as long as it is wanted

Our carers focus on comfort, dignity and calm. Keeping someone clean and comfortable, managing the practical side of the day, and being a steady presence in the house.

We are not a nursing service. Symptoms and medication are managed by the district nurses, the GP and the palliative care team, and we work alongside them so everyone is following the same plan.

What we provide is the day-to-day care that makes staying at home possible: personal care, help with eating and drinking for as long as it is wanted, and someone in the house so a family can rest.

What palliative care at home includes

Every situation is different, and the plan is agreed with you and with the nursing team. It usually covers:

  • Personal care focused on comfort and dignity, at the person's pace
  • Help with washing, dressing, mouth care and keeping skin comfortable
  • Support with turning and positioning, as advised by the nurses
  • Help with eating and drinking, for as long as it is wanted
  • Medication prompts where they are set out in the care plan
  • Keeping the room, the bedding and the laundry fresh
  • Practical help around the house so the family can be present
  • Quiet company, and time so nobody has to sit alone
  • Daytime visits, waking nights or continuous care as things change
  • Working closely with district nurses, the GP and the hospice team

Support for the family

Families take on a great deal at this point, and much of it is invisible. Broken nights, lifting, worrying about getting it wrong, and rarely being able to leave the house.

Our carers take the practical weight so you can spend the time as you want to spend it. That might mean we do the personal care while you sit and talk, or that we take the nights so you sleep.

You can ask us anything, including the things that feel difficult to ask the nurses. If we do not know, we will say so and find out.

Nights, and care that does not stop

Nights are usually what breaks a family. Getting up every hour, listening out through the wall, or sitting up because nobody can face the alternative.

We provide waking nights, where a carer is awake and with you through the night, and continuous care where support is needed at all hours. Cover can often be arranged within a day.

If things change quickly, tell us. Increasing care at short notice is a normal part of this work, not an exception we grudgingly make.

Afterwards

When someone dies, our carers do not simply stop and disappear. If we are with you, we will help with the immediate practical things and stay until you have someone with you.

In the days afterwards we withdraw gently, sort out anything outstanding, and we are happy to talk if it helps. Cruse Bereavement Support, on 0808 808 1677, is there for longer term support.

Common questions

How quickly can care start?

Often within a day or two, and sometimes the same day where a discharge is arranged. Call us with the timescale and we will tell you honestly what we can do.

Can you provide overnight or 24 hour care?

Yes. Waking nights, sleeping nights and continuous care, and we can usually increase cover at short notice as things change.

Do you work with the district nurses and hospice team?

Always. They manage symptoms and medication, we provide the daily care, and we follow the plan they have put in place.

Do your carers manage pain or give end-of-life medication?

No. That is done by the nurses and the GP. Our carers prompt medication where the care plan says so, and they contact the nursing team promptly when something changes.

What support is there for the family?

Practical care so you can be present rather than exhausted, nights so you can sleep, and someone to ask questions of. We can also point you towards bereavement support.

What happens after someone dies?

We help with the immediate practical things and stay until you have someone with you. Afterwards we withdraw gently, and we can point you towards longer term support.
