Key Duties:

  • Scheduling and Coordination: Plan, manage, and allocate care staff to clients based on individual needs and staff availability, ensuring continuity of care.
  • Client Relations: Act as the primary point of contact for clients and their families, addressing concerns and updating care plans as required.
  • Team Supervision: Support and oversee care staff, providing guidance, resolving issues, and monitoring
  • Compliance Management: Ensure all care services are delivered in line with company policies, CQC standards, and other regulatory requirements.
  • Record-Keeping: Maintain accurate records of care schedules, client feedback, and staff performance using care management software.
  • Onboarding and Training: Assist in recruiting, inducting, and training new care staff to meet the company’s quality standards.
  • Crisis Management: Respond promptly to emergencies, adjusting schedules and reallocating resources as needed.

Job specific requirements:

  • Experience in Care Coordination: Proven experience in a care coordination or similar role within the healthcare sector.
  • Knowledge of Care Standards: Strong understanding of CQC regulations and best practices in care
  • IT Proficiency: Competency in using scheduling and care management software, as well as Microsoft Office applications.
  • Organisational Skills: Ability to manage multiple tasks, prioritise workload, and adapt to changing

Qualifications:

  • NVQ Level 3 (or higher) in Health and Social Care (or equivalent). Additional qualifications in care management or leadership (desirable). Personal Skills:

• Health or Social Care qualification or CLAiT Award.
